What to Expect?

You can expect your initial eligibility visit to take about an hour.
The purpose of your initial visit is to:

  • perform a comprehensive assessment of your current situation
  • determine which of our services can help you
  • determine your eligibility for those services
  • determine any outside services that could benefit you, and initiate any referral process those services require

Our Skills & Expertise

Case managers work closely with their clients, medical providers as well as partners and loved ones (when appropriate) to develop and implement a plan of care, to coordinate medical resources, communicate health care needs, and monitor each client’s progress. In addition to meeting the health needs of the clients, Medical Case Managers are the clients strongest advocate when working “the system” to identify additional resources and support needed to meet the human services needs.

Case Management helps people living with HIV navigate their care and the support systems available in the West Central Texas Area. They are trained to respond to the needs of people impacted by HIV/AIDS and help plan for long-term needs. Case Managers help guide you to accessing important services, support, education, and staying involved in your healthcare.

Documents Checklist

Please bring as many of the following documents as possible. If you do not have all the required documentation, our Care Coordinators will assist you in completing the list.

Identification

(ex: Texas ID, LUPE ID, Matricula Consular, Tarjeta de elector, or TDCJ ID)

Proof of Residence

Utility Bills, Lease Agreement, Mortgage Statement, etc.

Proof of Income

3 months worth of Pay stubs, last year’s W2 forms if employed, if unemployed, a supporters statement can be provided to you, Social Security Income Award Letter

Proof of HIV status

 Your labs or medical records and a list of any current medications.

Insurance, Medicaid, Medicare, etc.

Copy of Medicaid, food stamp, ADAP or other private insurance card, if you have them. (Insurance is not required to apply for services)
